Perley Health is pleased to share that we have been selected as one of three Nursing Home Without Walls pilot sites in Ontario.  Nursing Home Without Walls (NHWW) is a community-based program that helps older adults age safely at home with access to essential services and support. Perley Health has been selected as one of three pilot sites in Ontario for the NHWW pilot program funded by AdvantAge Ontario. 

Perley Health's Nursing Home Without Walls program is partnering with the seniors committee of the Alta Vista Community Association – Senior Watch Alta Vista (SWAV) – and its Naturally Occurring Retirement Community (NORC) initiative and provide a coordinator to support the SWAV-NORC community in accessing services to age in place.   

In preparation for the program launch, we would like to hear from you about your needs as you age in place in the community. The purpose of the survey is to better understand the needs and priorities of older adults in our community who wish to age in place and may be eligible to access FREE services through this partnership.

Perley Health is proud to announce its selection as one of three pilot sites in Ontario for the Nursing Home Without Walls pilot program. Funded by AdvantAge Ontario, this innovative model extends Perley Health expertise into the community.

Our mission is to empower Veterans and Seniors to reach their full potential at any stage of the aging process - whether they reside within our long-term care home or in the broader community.

What is Nursing Home Without Walls?

Developed originally in New Brunswick, the Nursing Home Without Walls model addresses the "crisis placement" trend by providing proactive, upstream supports before seniors reach a point of emergency or avoidable hospitalization. This person-centred program is free for participants and tailors services to an individual's cultural context and specific health goals.

Why This Matters

With more than 1,200 individuals currently on the waitlist for long-term care at Perley Health, we recognize the urgent need for support today. This pilot allows us to reach these individuals earlier, assisting them to live safely and independently at home for longer.

This program is a key component of our strategic objective to double the number of people we serve by 2035. By expanding our "community of care" model, we ensure that more Veterans and Seniors in the community benefit from Perley Health's expertise.

We know that a senior's well-being is deeply connected to their family and caregivers. Nursing Home Without Walls offers families "confidence and assurance" by providing professional care that reaches their loved ones at home, reducing the stress and situations that lead to emergency hospitalizations. 

Responsible Innovation

Perley Health is committed to implementing this pilot responsibly and transparently.

  • This project is funded by Healthcare Excellence Canada and The Waltons Trust.

  • We are hiring a dedicated Program Coordinator, based out of our Active Seniors and Veterans Learning Centre, to engage directly with the community.
