Perley Health provides various types of long-term, convalescent and respite care. Home and Community Care Services Champlain (formerly Champlain Local Health Integration Network) determines eligibility for our care programs. Note that pandemic restrictions forced us to temporarily suspend some of our care programs and tours.

Admission Process

Home and Community Care, Champlain Local Health Integration Network (LHIN) is responsible for determining eligibility.
