Over the next two decades, Canada must accommodate the needs of another four million seniors — eighty-two per cent of whom will be 75 years of age and older. SeeMe® is evidenced-informed and has been formally evaluated at Perley Health.

What is SeeMe®?
SeeMe® is all about enhancing quality of life through each person's journey. It starts with understanding frailty. Instead of a one-size-fits-all approach, SeeMe® prompts the essential questions:
- What are the individual's quality-of-life goals?
- How can we honour individual wishes if there is a health change?
- What lifestyle preferences should be considered?
- Which medical intervention is best aligned with the individual's goals of care?
- How does frailty impact the outcome of medical procedures?

Discuss Goals of Care During Acute Health Events: An E-module for Nurses in LTC
Empowering long-term care staff to confidently navigate acute health events with residents' goals of care is the focus of this new interactive training e-module. Developed collaboratively with nurses, residents, and subject matter experts, this training is built upon the principles of the SeeMe®: Understanding frailty together framework, a creation of Perley Health.
This training e-module:
- Equips long-term care nurses to prioritize resident-centred care during critical moments.
- Helps to ensure that staff are prepared for challenging conversations and decisions that can result from acute health events.
- Grounds learning in resident values, as promoted by the SeeMe® framework
- Facilitates collaborative care planning that respects individual preferences and lifestyles.
- Allows organizations to elevate knowledge, skills, and confidence in leading discussions on goals of care, ultimately improving the care provided to residents.
- Is available for free as a standalone learning module or as part of the SeeMe® training and education suite. It can be integrated into corporate learning solutions and Learning Management Systems.

These just-in-time bedside tools for hip fractures are part of a broader Acute Health Event Management resource that provides high-level clinical information for managing six common diagnoses in long-term care. They focus specifically on supporting your team to manage a hip-fracture more confidently. These pathways emphasize pain management, diagnostic considerations, symptom management, care planning, and compassionate end-of-life care, all tailored to the unique needs of long-term care residents.
Our structured, step-by-step hip fracture pathways promote interprofessional collaboration, ensuring physicians and allied health are involved in holistic decisions aligned with resident wishes. It prioritizes open communication with residents and their families, fostering trust and ensuring their choices are respected. Furthermore, it emphasizes the importance of reviewing and updating end-of-life care plans, ensuring person-centered care remains at the forefront.

6 Key Elements of an Effective Skin and Wound Care Program
In 2021, Perley Health, once the worst in the province for skin and wound care, undertook a dedicated and transformative journey to become a beacon of excellence. This remarkable shift hinged on six key elements introduced with unwavering commitment and determination. By the end of 2021, Perley Health celebrated a 52% reduction in worsening pressure injuries and a 56% decrease in skin and wound infection rates, ultimately making it a model of sustainable excellence in skin and wound care. The KT Hub is part of the overall strategy of the Centre of Excellence in Frailty-Informed Care™.
Perley Health's Centre of Excellence in Frailty-Informed Care™ (CoE) has developed a Knowledge Translation Hub to facilitate the dissemination of our applied research projects, and to support the implementation of best practice into the context of long-term care (LTC) by collaborating with partners and other LTC homes. The last few years have highlighted the resiliency and innovation that already exists within the long-term care community. Research shows that LTC seeks out advice on care improvement to both regional and provincial colleagues. We want to help amplify these successes. LTC is a unique care context and therefore has specific barriers and facilitators to successfully implement best practice. Training and education and well-designed strategies are recommended as key interventions to support the adoption of innovative care ideas in the LTC context.
