Volunteer Award Nominations
It's time to recognize exceptional Perley Health volunteers!
As we prepare for National Volunteer Week 2024, nominations are now open for the Volunteer of the Year, Volunteer Advocacy, and Volunteer Team Awards. Please use the attached nomination form to tell us why you think a particular volunteer, or group of volunteers, deserves to be recognized this year. #EveryMomentMatters when you're working to elevate care for seniors and Veterans- let us know how Perley Health volunteers made a difference!
Nominations are due by March 22nd.
All staff, residents, family members, and volunteers are invited to submit a nomination by emailing the completed form below to volunteer@perleyhealth.ca or dropping it off at Volunteer Services (located near the Main Reception at Perley Health).
